    An Attempted Suicide with Copper Sulphate injected intravenously:Pathopsysiology and Therapy about a case report

    Acute copper sulphate poisoning is an unusual event, rarely following parenteral exposure, complicated by toxicological effects as haemolytic anaemia, methaemoglobinaemia , hepato-renal damage and acute rhabdomyolysis. Currently, the therapeutic management ignores unique classes of evidence and is mainly based on supportive and chelation therapies. Case details. This case report describes acute copper sulphate poisoning in a 37-year-old man who attempted suicide by self-injecting an unknown amount of copper sulphate. Within three days the patient developed severe intravascular haemolysis and rhabdomyolysis. Initial therapy relied on intensive supportive care with fluids administration, electrolyte correction and packed red blood cells transfusion. The D-penicillamine (30 mg/Kg/day per os) was prescribed as chelation therapy by the poison control centre. The N-acetilcysteine and ascorbic acid were administered to prevent further oxidative stress. Later on, two sessions of therapeutic plasma exchange were performed in order to support the drug therapy. Haemolysis and rhabdomyolysis reversed throughout the hospital stay. Discussion. In this case the antidotic and antioxidant therapy resulted effective to reverse haemolysis and rhabdomyolysis and to prevent hepatic and renal damage. Moreover, this case underlies that therapeutic plasma exchange should be considered as an additive measure to undertake since the earlier stages of the emergency intervention.     The Rock Mass Quality Index (RQI): a quantitative tool for the quality evaluation of near-surface rock masses

    The knowledge of rock masses behaviour is an important information in various fields such as civil engineering, land use planning and hazard/risk zoning. Different rock mass classification methods, initially aimed at assisting underground excavations (Hoek, 2007), are widely used nowadays for preliminary design procedures (Bieniawski, 1989; Hoek, 2007), like the RMR (Bieniawski, 1976) and the Q (Barton et al., 1974) and their modifications. These methods incorporate geological, geomechanical and geometric parameters in order to obtain a quantitative estimation of the rock mass quality, but, on the other hand, their implementation is time-consuming. Despite the dominance of these two methods, further rock mass classifications systems have been proposed in the last decades and, among these, the Geological Strength Index (GSI) classification system is currently widely used as it allows to estimate the strength of rock mass through empirical semiquantitative evaluation (Hoek, 1994; Cai et al., 2004), based on both rock mass structure and condition of the joints (Hoek et al., 1995). Estimating the GSI is straightforward and fast, but it comes at the cost of a certain degree of subjectivity. Moreover, the index does not adequately account for the lithology of the rock mass matrix. Hence, for the above reasons, these classification methods are not fully suitable to collect rock mass data over wide scale areas for engineering geological mapping. The Rock mass Quality Index (RQI, Disperati et al., 2016; Mammoliti et al., 2018) is a rock mass classification system developed for cartographic purposes and it is based on the systematic fieldwork measurement and processing of sets of the Schmidt hammer rebound values (R). Each representative rock mass outcrop is analysed by collecting ca. 20 R values at the 15-25 nodes of a regular grid conceived to investigate the typical features of the rock mass. This allows to perform statistical analyses and to calculate the RQI, a quantitative indicator of the global strength and quality of the rock mass. In the last decade, a dataset of ca 1100 outcrops sites spreading over a large area (ca. 12000 km2) were acquired in Tuscany (Italy), according to different lithology, weathering, jointing conditions. The dataset consists of both RQI measurements and GSI estimations for the main different lithological groups (flysch, limestones, marls, magmatic rocks and schists) of the Northern Apennines (Italy), as well as the laboratory determinations of the Slake Durability Index (Id2; Franklin & Chandra, 1974) obtained by testing representative outcrop rock samples. The large dataset has allowed to analyse the correlation among RQI, GSI and Id2 and to perform an in-depth critical analysis of the relationships among RQI, lithology, rock mass structure, as well as the suitability of the RQI as reference index for engineering geological mapping of near-surface rock mass quality

    Engineering geological mapping of near-surface rock mass quality of folded and thrusted arenaceous flysch units in the Northern Apennines (Italy)

    The regionalized knowledge of the quality of near-surface rock masses is an important tool for land management/planning, as well as for guiding further in-depth studies aimed at landslide and earthquake risk assessment and civil engineering planning. The characterization of heterogeneous rock masses like flysch units represents a relevant challenge to engineering geologists due to the complex structure of these materials, which results from both their depositional context and tectonic history. Flysches are widespread all over the Apennines chain and their mechanical characterization is a difficult task given the occurrence of intercalation of layers with different lithology and strength. Moreover, the complexity of the thrust and fold tectonic framework makes the regional distribution of these characters difficult to predict. The aim of this work is to provide a method to map the near-surface rock masses quality for an arenaceous flysch widely cropping out in the outer Northern Apennines (Torrente Carigiola Formation, Aquitanian; Bettelli et al., 2002). This formation is mapped in both the geological map of the Regione Toscana (Italy) at the scale of 1:10,000 and the geological sheet “252 – Barberino di Mugello” (Bettelli et al., 2002) of the Italian Geological Map at the scale of 1:50,000 (CARG). It is made up by intercalated arenaceous (A) and pelitic (P) layers characterized by variable A/P ratio. The rock mass quality is evaluated by estimating, for a set of representative rock outcrops, the Rock Mass Quality Index (RQI; Disperati et al. 2016; Mammoliti et al. 2018). This index results from the analysis of both systematic Schmidt hammer rebound measurements (R) acquired at the nodes of a regular grid (ca. 20 R measurements for ca. 15-25 nodes) and the determination of the unit weight for representative outcrop rock samples. For the same outcrops, also the A/P ratio and bedding attitude are determined. The results show a positive linear correlation between RQI and the A/P ratio, confirming that the latter parameter is an important feature controlling the rock mass strength. This correlation is used to assess the distribution of both parameters within a set of geological cross sections traced normal to the regional structures trend (main thrusts and km-scale folds). Then, the structural features available from the literature geological maps allow us to extrapolate both the RQI and A/P ratio from the profiles to the map scale. Finally, a further set of the same rock outcrop data acquired after the above-described modelling procedure is used to check the accuracy of the method

    A psychometric modeling approach to fuzzy rating data

    Modeling fuzziness and imprecision in human rating data is a crucial problem in many research areas, including applied statistics, behavioral, social, and health sciences. Because of the interplay between cognitive, affective, and contextual factors, the process of answering survey questions is a complex task, which can barely be captured by standard (crisp) rating responses. Fuzzy rating scales have progressively been adopted to overcome some of the limitations of standard rating scales, including their inability to disentangle decision uncertainty from individual responses. The aim of this article is to provide a novel fuzzy scaling procedure which uses Item Response Theory trees (IRTrees) as a psychometric model for the stage-wise latent response process. In so doing, fuzziness of rating data is modeled using the overall rater's pattern of responses instead of being computed using a single-item based approach. This offers a consistent system for interpreting fuzziness in terms of individual-based decision uncertainty. A simulation study and two empirical applications are adopted to assess the characteristics of the proposed model and provide converging results about its effectiveness in modeling fuzziness and imprecision in rating data

    The NILUS reactor for the co-production of electricity and potable water

    The NILUS-1000 reactor, an innovative PWR with integrated layout and full natural circulation, is proposed for the adoption in those countries where the production of potable water is a major concern for the population, and where the nuclear culture could not be so developed and firmly established as the complexity of the nuclear industry would require. The NILUS concept is briefly depicted, together with the reference desalination process and plant size selected for the coupling with the 1000MWth nuclear reactor. The safety systems and the “hybrid” containment, a compact steel pressure containment with a pressure suppression pool, are described. The results of the preliminary safety analysis are reported for an ATWS accident, showing the validity of the concept design and the large grace period achieved in this beyond DBA transient

    “A case of ecological renaturation in a drained Mediterranean peatland: the case study of the Massaciuccoli Lake basin (Tuscany, It)”

    The Massaciuccoli Lake floodplain is located in the Natural Park of San Rossore, Migliarino and Massaciuccoli in NW of Tuscany (Italy). Since the 1930s, a large part of the Massaciuccoli floodplain has been drained for agricultural purposes. To ensure a water table depth suitable for cultivation, a complex network of artificial drains and pumping stations has been used to drain the superficial aquifer and rainwater. Land use is characterised by conventional agriculture (covers 80% of the area) and periurban infrastructures, such as a wastewater treatment plant. In the peatland area, cropping systems are based on continuous production of maize (Zea mays L.), sunflower (Heliantus annuus L.), wheat (Triticum spp. L.). As a consequence of land use, several environmental concerns arose in the last 50 years. The most important concerns are those related to: I. eutrophication of the lake due to nutrient enrichment (N, P) in the surface- and groundwater. II. the subsidence rate (2-3 m in 70 years) due to compaction and increased mineralization of peat. The project RestoMedPeatland (https://sites.google.com/site/restomedpeatland/) started in 2011, identified rewetting and setting-up a phyto-treatment system as the solution for improving water quality,slowing down soil organic matter (SOM) mineralization, and, therefore, a method to restore the ecological functions of this site. A 15 ha experimental area was used to compare the efficiency of three different systems (A: constructed wetland, B: paludiculture system and C: natural wetland in treating the eutrophic drainage water coming from a sub-watershed in the reclamation district.In the natural wetland (C), after top soil removal, excavation and rewetting with drainage water, the vegetation has evolved naturally. The surveys carried out during the four years after pilot experimental field, highlighted the development of a spontaneous hydro-hygrophilous vegetation. This was mainly composed of helophytic communities dominated by Phragmites australis (Cav.) Trin. and Typha latifolia L., in addition to monophytic populations of Myriophillum spp., occupying different areas of the system according to the depth of excavation, for about 50% of the whole surface. The flora, currently, consists more than 30 species, as hydro-hygrophilous Alisma plantago-aquatica L., Juncus effusus L., Apium nodiflorum (L.) Lag, Lemna minor L., characteristic of the nearby Massaciuccoli Lake. The data collected so far showed a high and progressive growth of the biomass of the natural area, a dynamic upward trend of floristic-vegetation biodiversity, and, from a technical standpoint, a large phyto-treatment capability

    The Sulcis Basin is an area situated in SW Sardinia (Italy) and is a potential site for the development of CCS in Italy. This paper illustrates the preliminary results of geological characterization of fractured carbonate reservoir (Miliolitico Fm.) and the sealing sequence, composed by clay, marl and volcanic rocks, with a total thickness of more than 900 m. To characterize the reservoircaprock system an extensive structural-geological survey at the outcrop was conducted. It was also performed a study of the geochemical monitoring, to define the baseline conditions, measuring CO2 concentrations and flux in the study site

    Plutonia-Thoria Fuel Cycle as Starting Solution for a wider Thorium Use

    This paper is focused on a description of thoria fuel option. Our opinion is that this option, beyond being a valuable way to exploit the energy content of plutonium without further breeding it, may be a starting point for introducing an Uranium-Thorium fuel cycle, based on a different strategic context with respect to past proposals. The option is based on the adoption of current or advanced PWRs, the latter characterised by a reduced fuel power density, always adopting conventional fuel rods and assemblies. A three-batches full core loading scheme is assumed. The thoria-plutonia composition is determined by the constraints to obtain at Beginning Of Life (BOL) a non positive void coefficient, and to reach a burnup as high as possible. Different fuel compositions and pellet radius are considered. The plutonium content is in the range of 4.5÷15%, mainly depending on the plutonium quality, namely Weapon Grade (WG) or Reactor Grade (RG). The results are in terms of dynamic coefficients, life duration, plutonium consumption and final isotopic compositions. These fuels show the capability to destroy about 40÷60% of total plutonium for RG, while this figure rises to 65÷70% for WG. These values are well above those obtained by MOX option. A variant to eliminate any proliferation concern foresees the addition of small quantities of 238U at the expenses of a reduction of the fuel burnup and its capacity in burning RG plutonium, while the opposite occurs for WG. The low boron worth is not different from the MOX one, being related mainly to the plutonium content, and much less to the chosen fertile isotope. Therefore, modifications of control devices for a full core strategy is not expected to be different in the two cases. The results confirm the viability of this proposal, apt to future variants, including those connected to accelerator actinide burning solution. An irradiation experiment, expe-cted to take place at Halden HWBR in the context of the ENEA participation to the Halden Project, is the main part of the Inert Matrix – Thoria fuel R&D activity presently underway as a Polytechnic of Milan–ENEA co-operation

    Onion under Microscope: An in-depth analysis of the Tor network

    Tor is an anonymity network that allows offering and accessing various kinds of resources, known as hidden services, while guaranteeing sender and receiver anonymity. The Tor web is the set of web resources that exist on the Tor network, and Tor websites are part of the so-called dark web. Recent research works have evaluated Tor security, evolution over time, and thematic organization. Nevertheless, few information are available about the structure of the graph defined by the network of Tor websites. The limited number of Tor entry points that can be used to crawl the network renders the study of this graph far from being simple. In this paper we aim at better characterizing the Tor Web by analyzing three crawling datasets collected over a five-month time frame. On the one hand, we extensively study the global properties of the Tor Web, considering two different graph representations and verifying the impact of Tor's renowned volatility. We present an in depth investigation of the key features of the Tor Web graph showing what makes it different from the surface Web graph. On the other hand, we assess the relationship between contents and structural features. We analyse the local properties of the Tor Web to better characterize the role different services play in the network and to understand to which extent topological features are related to the contents of a service

    Measuring Airway Obstruction in Severe Asthma in Children

    Lung function is an important tool in the diagnosis and monitoring of patients with asthma at all ages. Airway obstruction is a typical feature of asthma and it can be assessed with several lung function techniques. Spirometry, respiratory resistance and reactance, and lung volumes are available to measure it at different ages and in children. The assessment of a bronchodilator response is always recommended to show the reversibility of the obstruction. Poor lung function is a predictor of poor asthma outcome and a low Forced Expiratory Volume in the first second of expiration percent predicted measured with spirometry, has been shown to be associated with a higher risk of having an exacerbation during the following year independently of the presence of asthma symptoms. In severe asthma lung function assessment is used to distinguish different phenotypes, children with severe asthma have worse airflow limitation prior to administration of a bronchodilator than children with non severe asthma. Airway resistance and reactance are indirect measurements of airway obstruction and they can be measured with the forced oscillation technique, which is feasible also in non-collaborative children. This technique can be more informative in discriminating patients with asthma from healthy controls and is able to indicate a more peripheral involvement of the airways. The role of this technique in severe asthma is still debated. In conclusion lung function is useful in the clinical management of children with severe asthma
